Klassic Soccer requires a team registration fee + $5 per player facility fee (for in-district teams) + $13 per player and coach OYSA registration fee along with the following forms:

-Team Entry form
-Roster (complete with names, addresses, and dates of birth)
-Individual registration form (one for each player, signed by parent)
-Team Payment form (in-district teams)
-Team Payment form (out-of-district teams) (teams outside of Eugene/Springfield)
-Field Availability form (for out of district teams home field info)
-Coach Conflict Request form (for coaches to request game scheduling preferences)
-Coach Disclosure form (for coach background check, needed once per year)

ALSO, once each school year every Klassic Soccer (4th-12th grade) coach must register themselves on-line and complete the background check info on-line HERE at this link!!!
